The North Kern - South Tulare Hospital District owns and operates Delano District Skilled Nursing Facility and the Gloria Nelson Center for Women and Children. The skilled nursing facility is a 141-bed facility that provides 24-hour nursing care to its residents. The Gloria Nelson Center provides OB-GYN to women of all ages and pediatric healthcare services to children 21 years of age and younger.

At Delano District Skilled Nursing Facility (DDSNF), we are committed to providing our residents with a great level of quality care. High standards and our commitment to excellence have led to an organization composed of friendly staff, cleanliness, and the dignity of our residents.   

Our goal is to create a sensitively paced program designed to meet the needs of our residents and their families. Our inter-disciplinary team will create a care plan specifically designed to achieve our resident's maximum potential and to restore his or her highest possible level of well-being. DDSNF provides care and opportunities for a either a short-term or long-term stay.

DDSNF is an advocate for senior citizens and strives to encourage independence with family involvement. However, often times, an elderly citizen may experience a decline in medical condition and can no longer make needs, wants, and concerns known to staff and family. In a perfect world, family would know exactly what their loved would want in regards to care and end of life decisions. Unfortunately, this is not always the case. We encourage discussions with family about end of life measures, code status, and appointing a family representative to make decisions when incapacitated. It is never too early to discuss or formulate an Advance Health Care Directive (AHCD). If the AHCD form is for a family member currently residing in a skilled nursing facility, an Ombudsman must be present for signature. You may coordinate this with our Director of Social Services.

The Gloria Nelson Center for Women and Childen (GNC) is a newly constructed 9,800 square ft. medical center awaiting rural health clinic designations by Licensing and Certification. This center has been created to provide excellent healthcare to women and children in this medically underserved area. The Board of Directors and Executive Staff have planned this healthcare development over the past several years ensuring that services will compliment and coincide with the existing healthcare facilities in Delano.
