Admission to Delano District Skilled Nursing Facility:

DDSNF is a 141 bed facility that provides 24-hour nursing care and can accommodate for either short term or long term stays for our residents.

New residents can be directly admitted to DDSNF from a hospital, a home setting, another skilled nursing facility, an independent living center, an assisted living center, or a residential care facility for the elderly (RCFE). Residents can only be admitted to a skilled nursing facility with a physician’s order from the resident’s primary care physician.

We understand that the decision to place a loved one in a facility for care can be a difficult decision to make. Our compassionate and trained Nursing, Social Services, and Activities staff are here to assist in the transition process and adjustment period. Our team is dedicated to making your decision be as easy and smooth as possible by guiding you and assisting you to make decisions that are in the best interest of your loved one.

DDSNF gladly accepts the following insurances to cover skilled nursing facility costs. However, please note that most health insurance plans do not provide for long term care unless it was specifically included at the time the health insurance was acquired. Many health insurance plans provide for short term care or a limited amount of days provided that the patient has had a 3-day qualifying stay in a hospital setting.

  • Medicare A
  • Medi-cal
  • Various Medi-cal HMOs
  • Managed Care (GEMcare)
  • Various HMOs (per facility contract)
  • Private Pay
